This form will let you modify your Skyrim character by creating the necessary console commands which could be run in batch. This only works for the PC version (unless there's some way to bring up the console screen and run a batch file in other platforms).

Notes
Modifying your character may break your game or have unintended consequences:
Select the minimum skill level required to your target character level otherwise you may not have enough experience points to reach level 81.
Some quests are level-dependent and you may not be able to get them until leveling-up especially if you modify your character from level 1 to 81. In my case, I started from level 78 (my level from my previous game which has become quite unstable) and I wasn't able to do Mjoll's quest until I leveled to 79.
Modifying your character may not work properly while you are a vampire or a werewolf.
Two batch files are needed for the modifications to work properly.
The first file will deselect all the perks and the second file will do the modifications.
Running the commands in one batch file will result in the selected perks not being displayed as selected and may not work (v1.3.10).
